Plan of Care
On our first visit to your home, our goal is to generate a Plan of Care customized to your loved one. The needs assessment, the time assessment, and input from family members are taken together to help us design the plan that will work best for you. If you haven't already completed the needs and time assessments, we'll do them with you during the first visit.
Remember this - we have the flexibility to work with your scheduling needs, with one or more caregivers, depending on what is required. Whether it is 4 hours per day for just one or two days a week, or round the clock care 7 days a week, we can meet your needs.
